Your sex is determined by your sex chromosomes and represented by which reproductive organs you have. Most people are male or female (exclusive), while a few people are both male and female by virtue of having the corresponding primary sexual characteristics of both.
I don't feel like getting into the "how many genders are there / what genders are valid" discussion, but I get pissed off when I see a "Gender" option on things like medical forms. Your doctor almost certainly doesn't give a fuck whether you identify as a male, female, or trigender amorphous potato. They need to know what your biological sex is so they know what treatments to administer and shit like that.
However, if someone feels like their gender isn't also their sex, I say: who cares? Just don't conflate the concepts.
